Simple Forms guide
Turn any PDF you already use — an intake form, a waiver, a rental agreement — into a form people fill in and sign on their phone. Every submission comes back as a completed PDF in your original layout.
Overview
Simple Forms is part of Credible Documents. You keep your existing paper form exactly as it is: upload the PDF, mark where the answers go, and share a link or QR code. People fill it in on any device, and you get a finished PDF with their answers (and signature) printed in the right places.

Creating a form
- Open Forms and click Upload PDF Template (or drop the file onto the upload area). PDFs up to 15 MB.
- The PDF opens in the form editor. Add a field, then drag it over the blank space on the page where the answer belongs and resize it to fit.
- Give each field a label (what you're asking for), an optional placeholder (example text shown in the empty box) and mark it required if it can't be left blank.
- Repeat for every page, then save. The form appears under Templates, ready to share.
Fields are positioned relative to the page, so they line up with your PDF on a phone, a tablet or a desktop screen alike.

Field types

| Dropdown | A fixed list of choices, so answers are consistent. Enter the options when you add the field. |
| Checkbox | Yes/no questions, accepting terms, opt-ins. |
| Signature | The person draws their signature; it's placed as an image in the box on the PDF. |
Tip: make each field the same size as the box on your paper form. Text is printed inside the field's area and sized to fit its height, so a very short field gives small text.
Sharing a form
Every saved form has its own public link and a QR code (in the Templates list). Use whichever suits the situation:
- Email or message the link to a client before an appointment.
- Put it on your website as a button or link.
- Print the QR code at the reception desk, on a clipboard on site, or on the counter — people scan it with their phone camera and fill it in there and then.
One form can be filled in by any number of people; each one creates a separate submission.
What the person filling it in sees
- Your PDF, with the answer boxes laid over it exactly where you placed them, and your name shown as the sender.
- They tap a box to fill it in, or step through the fields with Next and Prev. A counter shows how many fields are done.
- Filled fields are outlined in green. Required fields left empty are highlighted in red, and the form can't be sent until they're filled.
- For signature fields they draw with a finger or mouse.
- Before sending, they enter their full name and email address, then tap Submit Form.
After a submission
As soon as the form is sent, YourProDesk produces the completed PDF: your original document with every answer printed in its place and the signature embedded where the signature field is. The submission records the answers, the submitter's name and email, the time and the IP address it came from.
Open the Submissions tab in Forms to see them, newest first:

The list shows your 100 most recent submissions.
Credits
- Forms without a signature don't use credits — collect as many submissions as you like.
- Forms that request a signature use 1 credit per submission, taken from your balance when it arrives.
- If your balance is empty, the submission is kept as Pending and you get an email straight away. Top up within 48 hours to retrieve the signed document — after that it is permanently deleted.
- Turn on auto-recharge under Account → Credits so signed forms never wait.

Managing forms
The Templates tab lists each form with its number of fields, number of submissions, whether it collects a signature, and when it was created. From there you can:
- QR — show and download the QR code.
- View — open the form as people will see it.
- Delete — remove the form so the link stops working.
To change a form's fields, open it in the editor again and save — the new layout applies to submissions from then on.
Tips
- Mark only what you truly need as required — people abandon long forms on mobile.
- Use dropdowns and checkboxes instead of free text wherever the answer is one of a few options.
- Test your form on your own phone before printing the QR code.
- Keep the original PDF clean: blank lines or boxes where answers go make the completed PDF look like a hand-filled original.
